Extreme Eyewear can process and fill
prescriptions for lenses for our glasses; however, Extreme Eyewear
does not write prescriptions for lenses and will only fill an order
for prescription lenses if accompanied by a valid prescription.
Mail or Fax your prescriptions to Extreme Eyewear making sure that
all fields are filled out in the prescription. Make sure your doctor
fills out the entire prescription including the P.D. (Pupilary Distance).
Please consult your doctor. Prescription Lenses are for single vision
only.
